4-H Enrollment Information for
2009/2010 Program Year
The deadline for El Dorado County 4-H Enrollment in order to be eligible for the 2010 County Fair is October 15, 2009.
Enrollment is as easy as 1-2-3!
1. Register online at https://california.4honline.com/.
- Read the detailed instructions for using the on-line system: Guide for New Enrollment , Guide to Re-enrollment
- Forgot your password? Click "I forgot my password" to get an email with a new password. The email goes to the address you entered in the registration system last year.
- You must have an email account to register. (List of Companies Who Offer Free Email Accounts )
- Do not print the forms in the on-line registration system; some of them are different than what El Dorado County uses.
- New adult volunteers: Before enrolling, contact the 4-H office at 530-621-5503.
2. Bring/mail your forms & fees to the 4-H County Office.
- Forms: Please print out the appropriate forms listed below to turn into the 4-H office; do not print the forms in the on-line registration system.
- Fees: $22/youth, $16/adult, $5 for Hotline newsletter to be mailed to your home. Please make checks payable to UC Regents.
- Mailing address: El Dorado County 4-H, 311 Fair Lane, Placerville, CA 95667
- Note: Your on-line registration status doesn't change from Pending to Active until the office has all of the completed paperwork and money.
3. Make copies of your signed medical release forms.
- Give one copy to your Community Club Coordinator.
- Give one copy to each of your Project Leaders.
4-H Enrollment Forms 2009-2010
Turn in these forms to the 4-H County Office to complete your registration:
- Adult Volunteer Self Disclosure Form
- Waiver of Liability (complete one per person, for both youth and adults)
- 4-H Hotline Subscription (only to receive the monthly newsletter by mail instead of email)
Give these forms to your Club and Project leaders (not the 4-H office):
